First Aid:
This incident log form is to be completed by the First Aider on duty when the incident occurs. There are copies of this form in the First Aid bags onsite.
This form is for ALL incidents including an incident where the rider remounts their pony and continues to ride. The form should be completed, signed, photographed and sent to tomfuller@gmail.com & bennachie@pcuk.org with urgency. Once the form has been emailed and you have received a response that the form has been received please destroy the hard copy.
